Description
This five-course certificate program prepares professionals for case management and related roles, training them to effectively support older adults and individuals with disabilities across the continuum of care. Learners build foundational knowledge in key aging and disability concepts, person-centered assessment, and the navigation of complex service networks. The program emphasizes best practices in care management, including understanding social determinants of health, cultural humility, and ethical considerations. Learners gain strategies for supporting smooth care transitions, reducing barriers to care, and promoting consumer choice, preparing them to deliver coordinated, high-quality services in various settings.
This 20 CE certificate differs from our Person-Centered Case Management Certificate (with 24 CEs) in that it is comprised of five courses, and does not have the additional course on person-centered planning (although, the concept of person-centered planning is referenced throughout this certificate.)
